Handbook
Child Care Card
A child care card must be completely filled out for each child in care. I must
have a current phone number to contact at least one parent immediately at all
times.
Operation Hours 7:30 am to 5:30 pm
My door will not open until 7:30 am. Children must be picked by 5:30 pm. Additional
charges apply for late pick up. A $5.00 fee is charged for every five minutes
after 5:30 pm. Children will be released from care if the parents are continuously late
and/or late fees are not paid. NO EXCEPTIONS MADE.
Admission Requirements
1. Child care card completed
2. Child must be up-to-date with immunizations at all time (unless a release is
signed)
3. Signed and dated contract
4. Two weeks security deposit
5. First weeks payment
6. Parent’s place of work information and social security
numbers
Security deposit
A security deposit is due at the time of enrollment for two weeks of childcare.
This is a non-refundable deposit, but
it will be used toward the last two weeks of childcare if a proper 30
days written notice has been given. This deposit will hold the child’s position
until agreed upon start date stated in the contract. The deposit is ensure long
term care for the consistency of all
children.
Medication
Over the counter and prescription medication will be administered to a child only
after a parent or guardian has given written permission. Prescription medication
must be in the original container and labeled with the child’s name and
instructions for administering. Ask your pharmacist to divide your child’s
prescription between 2- labeled containers, one for home and one for daycare.
Tuition and Fees
FULL PAYMENT IS DUE EVERY WEEK REGARDLESS OF HOLIDAYS OR ABSENCES
Online payment is an option. If paying by check make checks payable to Little people, Big Minds LLC
Payment is required on MONDAY MORNING.
A $30.00 late fee shall be added if
payment is not received on Monday
morning.
*Payment is required for each week regardless of holidays
or absenteeism. Tuition must be paid prior to vacation or any other absenteeism
or the deposit will be used and the child’s position will be filled. I can not
hold any position open indefinitely due to maternity leave, extended illness,
vacation, or any absenteeism. If the parent chooses not to send a child to
school for an extended period, payment is still required to hold the child’s
position. A $35.00 fee will be charged for a NSF Check. If checks continue to
bounce, payment will have to be in cash.
Holidays
I will be closed the following holidays:
New Years Day, Good Friday,
Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Friday after
Thanksgiving, Christmas Eve, Christmas Day.
*if a holiday fall on a weekend the closest day to the holiday will be our holiday.
For example: if Fourth of July is on a Saturday, school will be closed on Friday.
Parents are still responsible for paying for these days
if they fall on a regularly scheduled day for their child. If Desiree Novak ever
takes a personal vacation, arrangements will be made for assistant teachers to
run the school during her absence.
Parents Must Provide
1. Two changes of clothes to remain in child’s locker at all times.
2. Diapers or pull-ups and any diaper ointment.
3. Dress child according to the weather forecast. We will attempt to go outside
everybody above 35 degrees.
4. Sunscreen if desired to be put on.
8. Weekly fees and late charges must be paid on time.
9. A 30 day written notice or 30 days payment upon termination of childcare
contract.
10. All medication for children.
Closing of the Daycare
Parent will be notifies at least 30 days in advance of
any date I will be closed except in the case of an emergency (I.E.- loss of
electricity, heat, water, natural disaster, flooding, death in the
family)
During a time of severe illness of the provider or any
family emergency, I will attempt to remain open with an employee providing care.
If circumstances shall arise that the daycare facilities will close, parents
will be notified as soon as possible. Parents are responsible for alternate
care.
Accident Procedure
In case of an accident, the assistant caregiver will be in charge of the children
not involved in the accident. 911 will
be called for extreme emergencies or the child will be taken to Mount
Clemens General or St Joseph. The childcare card will be taken to the hospital
with the provider in charge. The parent’s will be notified as soon as possible.
Termination of Contract by Parents
Parents must provide 30 days written notice to terminate
the contract or 30 days payment is still required. If proper 30 days written
notice is given the two week deposit will be applied to toward the last two
weeks of childcare otherwise the deposit is forfeited.
Termination of the Contract by Desiree Novak
I reserve the right to terminate the contract without prior notice for the
following reasons:
1. Non-payment of fees due.
2. Continuing late payment of fees.
3. Extended absence or illness of child without payment of weekly fees.
4. Continuing disruptive and/or harmful behavior from the child/children.
5. Continuing disruptive problem from the child/children.
6. Failure to follow any of the policies and/or procedures in this handbook and/or
signed contract.
Every attempt will be made to correct any problems that
occur before terminating the contract. The children we care for are all in long term care. I would like parents to know that at no time will your child be released from daycare without an excellent reason. Usually, it would be a reason listed above, however, I remade serve the right to terminate the contract for any reason deemed necessary. Again every attempt will be to resolve any problems that occur before termination. Because this is a business, I have to look out for the best interest of ALL the children involved in daycare as well as the staff.
